![]() ![]() Violation Observed: Provide 1 evacuation crib for every 4 non-ambulatory children throughout the center. 3A:52-6.4(b)(1) The center shall provide sleeping equipment for children 12 months of age and younger, the center shall provide for each child a crib, that complies with the Consumer Product Safety Commission's (CPSC) Federal Safety Standards for Full- Size and Non-Full-Size Baby Cribs and 1220, which is incorporated herein by reference, and can be obtained through the CPSC's website at Education-Centers/cribs, playpen, or other Office of Licensing-approved sleeping equipment that meets the following requirement that In room 5, an infant was placed face down in their crib by staff. Violation Observed: Ensure that children under 12 months of age are placed in a face-up position in their sleeping equipment. 3A:52-6.4(a)(1) For early childhood programs, the center shall provide opportunities for daily rest and sleep as follows: for children 12 months of age and younger, the center shall ensure that each child is initially placed in a face-up sleeping position unless a different position is indicated in writing by the child's health care provider. Violation Observed: Cease the use of pacifier straps or other types of attachment devices throughout the center. 3A:52-6.4(i) Children shall not use pacifiers with straps or other types of attachment devices. The center advised that the canopies were being used to keep children from jumping out of their cribs. The canopies have long strings and very loose material that could be hazardous to children. In room 6, there was 1 staff with 10 infants ages 0 to 18 months. ![]() ![]() In room 16, there was 14 children ages 18 to 30 months with 2 staff when 3 staff were needed. In room, 1 there was 1 staff with children before a 2nd staff came in the room. Violation Observed: Ensure that adequate staff/child ratios are maintained at all times throughout the center. 3A:52-4.3(c) The following staff/child ratios shall apply, except as specified in 3A:52-4.3(d) through (f):Age Staff/Child RatioUnder 18 months - 1:418 months up to 2 ½ years - 1:62 ½ years up to 4 years - 1:104 years - 1:125 years and older - 1:15 Violation Observed: Cease swaddling infants in room 5 where 2 infants were being swaddled in sleep sack with swaddle components. 3A:52-6.4(a)(3) For early childhood programs, the center shall provide opportunities for daily rest and sleep as follows: swaddling of children during rest and sleep is prohibited. Ensure the exit path leading down the stairs from the second exit on the center's 2nd floor is free of storage at all times. Violation Observed: Ensure that all exits and egress areas are unobstructed and that the emergency exit doors are easily operable.
